I made this about two years ago with an older version. It was recently fixed of errors. It is only a skeleton of a simple form, but could be made to include many entries by the user. Let me know if it will work for you.

chmod i beleive is 755. I can not check anything else until this eve or about 8 hours from now. On the malk.php you change the fist three lines only. There may be a chance that the malk is corrupt. Make sure you upload it in ASCII, not binary. Consult you ISP and make sure that they accept PHP. Generaly it is something simple. I also included a different version of the fun file.

yes it's work but i confuse why i delivery email from adhisun2000@yahoo.com, but in email jgtranstion.com from adhi@jgtranslation.com too

Do you mean that you recive the php sent email to both
adhisun2000@yahoo.com and adhi@jgtranslation.com? Double check the php file? Did you change anything in the fun file? The fun file should not need anything changed. You can however make change the cosmetics of the fun file so that it matches your own site.
