NEED A FORM THAT ACTUALLY WORKS

i've been looking for a form that i can use inside my flash movie as a way for potential clients to contact me. i've seen this done many times, and i've found numerous form tutorials and open source files. every one that i've tried hasn't worked properly for me. i never get the email from the formmail CGI script in my cgi-bin.

currently, my solution is that i've created a separate popup window with an HTML form that works perfectly. if anyone knows of a form that works please let me know where to download it.
