Can anyone help me make this form work? I need a form that looks and acts like this, which I can capture the variables and send them to PHP. I also want it to TAB thru accordingly.

I would love to use an existing form, but I already searched thru all the forms on Flashkit and none of them are that good. Also, none of them contain drop-down menus and/or have a very professional look to them. Thanks for the reply though man.

The one that you currently have is ****ed up!!
All the fields dropdown menu have been resized out of poporation.
