I agree 100% with your points, and it really does come down to the lighting issue. The blood splatter is just that, gerbs, overlay(ed).
The burn/smudge/dodge tools are probably the ones I overlook the most.
The biggest problem I have is just trying to finish an image. I spend alot of time working on it, and just want to post it rather than really getting the details down. Or, sometimes, i spend so much time on an image, and get stuck with whatever I ideas initially came up with rather than thinking about other possibilities (see March Madness, it's exactly what I did).
So, for any 'student' of photoshop reading this, take your time, leave the computer for a while, come back after a while, and critique your image more. Then add/subtract anything that needs to go in the image. Utilize ALL tools available to you, brushes and blending only get you so far.
