your problem is caused by a movieclip being given a Graphic behaviour.
open formonly.fla
double click into the instance of reservation-movie
in the Properties panel, for the instance of reservation form you have the behaviour set to Graphic.
change this to a Movie Clip behaviour and the file will start working :D
