I'm using swishmax 4 and want to setup a contact form. I'm new to MySQL and contact forms and need some help/advice..... directions

My web hosting plan (godaddy) says I have MySQL Databases support so I hope I can do this. Contact forms are possible in MySQL, right??

Can someone help me out pointing me in the right direction, maybe with a swi file I can get and modify to work for me.... and of course asks lots of questions probably.
